Cardiac Rehabilitation scheme
What is Cardiac Rehabilitation?
Cardiac rehabilitation is a programme of education and exercise sessions which aims to help patients with heart conditions to improve their lifestyle and their heart health. They help patients who:
- have coronary heart disease
- have had a heart attack
- have heart failure
- are recovering from heart surgery.
Am I eligible for the Waverley Phase 4 scheme?
You are eligible if you have completed phase 3 cardiac rehabilitation. Hospitals and GPs can refer patients to a qualified cardiac specialist, to complete their phase 4 programme in a supervised environment.
What do I do to join the scheme?
- Obtain a referral from your hospital or GP.
